Where to Mail Tax Forms?

Use the following "Plus-4" numbers to get returns and payments routed to the correct office within the department:

80261-0004 notice of deficiency, tax protests, delinquent tax payments
80261-0005 individual income tax, amended income tax, real estate conveyance forms
80261-0006 partnership, corporation, fiduciary income tax
80261-0007 severance tax
80261-0008 estimated, extension and e-file income tax payment coupons/vouchers
80261-0009 withholding tax, annual withholding reconciliations and miscellaneous business taxes (e.g., waste tire, fuel, heavy truck sales, aviation)
80261-0013 sales tax, use tax, county lodging, county short term rental, local marketing district tax
80261-0016 motor vehicle, driver licenses, motor vehicle registrations
80261-0017 driver's license Renew by Mail applications

Having Trouble Opening or Printing Files?

- You will need the Adobe Acrobat to open, view and print our PDF files.
- Your Internet Service Provider (ISP) may be experiencing heavy traffic.
- Also, some ISPs (like AOL) prevent you from viewing these files.
- You may want to try opening your browser in a new window after your connect to
the Internet using Internet Explorer or Netscape.
- You can also try contacting your ISP provider to help you configure your
Internet to allow these files.
- Some of our PDF files are very large and may take long to download, please allow
enough time for them to open.

The form looks fine on the screen but prints out incorrectly text appears as squares or has blacked out sections).

- Make sure you have the most recent version of Adobe Acrobat.
- When selecting the Print option a Print Dialogue Box appears. On this box click
on the check box Print As An Image
- Reboot your printer and try again
- Update your printer driver. Most printing manufacturers provide free printer
driver updates from their Web sites.

My screen goes blank and nothing shows up. Or, when I click on the link nothing happens.

- Try scrolling down with the vertical scroll bar. The form may be at the bottom
of the page.
- The plug-in between the browser and the Adobe Acrobat is not working. This could
apply to any type of browser. To correct this, make sure you have the most
recent version of Adobe Acrobat.
- Also, some ISPs (like AOL) prevent you from viewing these files. You may want to
try opening your browser in a new window after your connect to the Internet
using Internet Explorer or Netscape.
- You can also try contacting your ISP provider to help you configure your
Internet to allow these files.
- Try viewing another PDF from another site.

My computer freezes or the form appears to be corrupted (text is small, large or jumbled).

- The memory on your computer may be too full. To resolve this reboot your
computer and try again.
- The file size may be to large for your Internet connection speed or your ISP.
Try downloading the PDF directly to your PC.
- If you still cannot get this form, you may contact us with a request to mail the
form to you.

I receive an error message that says one or more of the following:

- Win32 Application Error
- Can not find Acrobat Plug-in
- Can not find Acrobat Reader
- Acrord32.exe cannot be loaded
- Error acrord32.exe illegal operation
- colorspace not found error
- Cs8 error

If you receive any of the mentioned messages when trying to open a PDF file, first make sure you have an updated version of Adobe Acrobat. Unfortunately, we cannot fix these types of personal computer errors. You can also check Adobe's Web site at www.adobe.com for help.

How else can I get the file?

You can download the file straight to your PC. To do this:

1. Right click on the PDF document you need.
2. Select "Save Target As" or "Save Link As" from the menu.
3. Save the file to a directory or folder on your computer.
4. Try to reopen the file from where you saved it.

You may contact us and we can mail the form.

Error on Income Tax Return

We cannot make any modifications to an income tax filing whether paper or electronic, prior to the filing being processed. Do not mail in a second income tax form or file a second electronic version. A duplicate filing could cause problems with your income tax account.

Once your original filing is processed, you should file a paper amended income tax return (104X) to add or change information on the original filing. Please check “My Income Tax Account” on the Web or by phone at (303) 238-FAST (3278) to monitor the processing of the original filing.
