Current Tax Forms
Most tax returns and forms can be downloaded from the LDR web site. Use the list of forms by tax year to ensure that you select the correct form for your tax period. Forms are available for the current tax year and the previous five years.
Most of the forms and exemptions certificates have fill-in data fields that enable the user to complete the form online and print it. Fill-In forms are designated with a pencil icon . To allow the fillable forms to be securely used on public computers, scripts have been added to delete the user’s personal data when the form is closed. If you need a copy of a form without the security scripts, see this FAQ under online forms for instructions on how to remove the scripts from the form or contact the Tax Administration Division Forms Section at (225) 219-2690.

Online Forms Questions
How can I download a form directly to my hard drive and stop the form from displaying in the web browser window?
* Use the "Save Target As..." (or "Save Link As...") browser option to
download the PDF file directly to your hard drive and bypass the Acrobat
Reader plug-in.
1. Go to the page containing the link of the file you are trying to save.
2. Move your mouse pointer over the link on the page.
3. Right click (or left click, if your mouse is configured for left handed
operation) on the link.
4. Click on "Save Target As..." (or "Save Link As...") when presented with
a menu.
5. Select a location on your PC to save the file, if you are not used to
saving files, save it to the desktop.
* Use Adobe Acrobat Reader to open the file after downloading. If you do not
have Adobe Acrobat Reader, you can download it free.
When I click on a form, the tax forms appear and then suddenly disappears. Why?
* Some PC pop-up blockers can prevent complete downloading of Louisiana state
tax forms associated with electronic filing. Your PC might be having this
problem if you have visited the LDR website and had displayed tax forms
appear and then suddenly disappear. There’s an easy way to solve this
inconvenience. Simply turn off your PC’s pop-up blocker prior to accessing
the LDR website.

What is the mailing address for individual income tax returns?
Individual Income Tax Returns That Include Payments should be mailed to the
following address:

All other Individual Income Tax Returns should be mailed to the following
address:

What do I do if I owe taxes, but am unable to pay by the due date?
Submit your return by the due date. By filing timely, you avoid delinquent
penalty. Any amount paid with your return will reduce the interest and late
payment penalty charges. The Collection Division will bill you for any tax,
interest, and penalties that are owed.
Do I need to file a Louisiana individual income tax return if I am due a refund?
Yes. Without your return and W-2 form, the Department will not know how much
tax credit to give you.
How can I register for Withholding Tax?
Contact Central Registration Section of the department @ (225) 219-7318. They
will send you an application to register for Withholding Taxes and assign a
Louisiana Revenue Account Number.
If I find I have remitted an incorrect amount of withholding, what should I do?
File an amended L-1 return for the period you are correcting. When filing an
amended return, please be sure to mark the box designed on the return to
ensure proper processing.
I would like to file my withholding tax returns every two weeks (bi-weekly). Can I register for semi-monthly filing frequency but file every two weeks?
No— Bi-weekly filing is not allowed. Semi-monthly filers file 24 returns each
year. Filing a return every two weeks will result in 26 returns filed each
year, which is two more than the semi-monthly schedule. The two extra returns
and accompanying payments will be rejected, which could cause the monies to be
misapplied with billings issued for deficiencies.
What do I do to set up arrangements to pay an outstanding tax liability?

How do I obtain assistance with my tax problems?
Tax assistance can be obtained from any of the LDR Revenue Offices. You may
also email your technical tax questions to LDR via the Tax Question Mailbox.
